The World Health Organization has declared the novel Coronavirus (COVID-19) a worldwide pandemic. Due to its capacity to transmit from person-to-person through respiratory droplets, the government has set recommendations, guidelines, and some prohibitions which Wholelistic Wellbeing, LLC., adheres to comply with the guidelines listed below. In consideration of my participation in the foregoing, the undersigned acknowledge and agree to the following:
I am aware of the existence of the risk on my physical appearance to the venue and my cooperation to adhering to the World Health Guidelines by understanding lack of not following the proper guidelines may cause injury or illness such as, but not limited to Influenza, MRSA, or COVID-19 that may lead to paralysis or death.
ESTABLISHMENT PRACTICES
Limit the number of employees per work shift.
Limit the number of client gatherings in the main lobby and stagger the clients appropriately.
Lobby area signage at the entrance stating that no one with a fever or symptoms of COVID-19 can enter.
FACILITY GUIDELINES
Clients and staff are required to wear the appropriate face coverings to be worn according to CDC guidelines-while in the facility at all times, to protect yourself and everyone else around you. Shields or chin guards are insufficient coverage and a mask must be worn in addition, to provide the appropriate protection.
Please wash hands upon entering the facility, prior to any treatments.
Hand wash often with soap and water for at least 20 seconds, especially after going to the bathroom, before eating, after blowing their nose, coughing or sneezing, or after touching high touch surfaces, e.g. doorknobs.
All staff and clients are expected to avoid touching eyes, nose, and mouth.
All staff and clients are expected to limit the use other workers’ phones, equipment and workspace when possible.
Practice social distancing by limiting close contact with other clients or members of the staff.
I ATTEST THAT:
Understand that clients and employees should stay at home or send home if they become sick while at work.
Pre-screen employees for fever and other symptoms prior to employees entering the building.
A person is considered recovered after they have been released from isolation by SD-DOH upon meeting the following: (1) at least 10 days have elapsed after their symptoms began, and (2) at least 24 hours have elapsed after their fever resolved and their other symptoms have improved.
I have not experienced symptoms of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.
I have not traveled to a highly impacted area within the UK in the last 14 days. I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
have not, nor any member(s) of my household, traveled by sea or by air, internationally within the past 30 days.
I did not, nor any member of my household, visit any area within the United States that was reported to be highly affected by COVID-19, in the last 30 days.
CLEANING & SANITIZING
Routinely and frequently disinfect high touch areas including entrance/exit doors, railings, counters, buttons on elevators, and pens at checkouts are conducted pre, during and post client visit.
Adequate hand washing supplies (hand soap, running water, disposable hand towels, waste cans, etc.) and hand sanitizer are all provided at the facility.
Post information on COVID-19 describing how to prevent the spread of germs are presented prior to each visit.
Alcohol-based hand sanitizers in public areas available for staff and clients and/or utilize touch less hand sanitizing solutions if possible.
Provide tissue, trash receptacles, and hand sanitizer wipes for carts or baskets provided in the restroom area.
Position a trash can near the exit to make it easy for employees and customers to discard tissues, paper towels, etc.
WHOLELISTIC WELLBEING
I hereby release and agree to hold Wholelistic Wellbeing harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the establishment, or that may otherwise arise in any way in connection with any services received from Wholelistic Wellbeing, LLC.
I understand that this release discharges Wholelistic Wellbeing, LLC from any liability or claim that I, my heirs, or any personal representatives may have against the wellness establishment with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Wholelistic Wellbeing, LLC.
This liability waiver and release extended to each client together with all owners, partners, and employees, is evident of the agreement of the policies listed above.
